The Career Foundation is currently seeking a Care Coordinator on behalf of one of our employer partners.

About the Opportunity

Salary Range: $51,836 - $62,458 based on experience, plus 4% in lieu of perks
Hours: 35 hours per week

Location: Based primarily at a community partner agency site.

Key Responsibilities:

· Assess client status by gaining an understanding of socio-economic position, psychological outlook, housing, financial and legal needs.

· Assist in locating required community resources by liaising with community groups and other health/social service agencies to provide referral information and act as client advocate.

· Act as client coordinator by ensuring all available and required resources are in place and used appropriately.

· Act as a resource person to staff, clients and community residents by compiling information on community resources and facilitating access to those resources by liaising with community groups.

· Participate in the design and implementation of new programs to meet individual client and community needs.

· Ensure comprehensive care for clients by providing information and short-term counselling, and when necessary, referring them to other professionals.

· As part of the circle of care, supports clients with their care plans, including Patient-Reported Outcome Measures (PROMs) and Patient-Reported Experience Measures (PREMs).

· Collaborates and delivers shared medical appointments and other wellbeing initiatives.

· Conducts intake interviews to ensure that clients meet the organisation’s mandate.

· Registers clients, collects socio demographic, health equity data and other relevant information, and conducts biennial updates of such data.

· Facilitates client advisory committees, client engagement and feedback mechanisms.

· Provide comprehensive introduction to all eligible services.

· Connects clients to appropriate community and social supports.





· Provides a range of instrumental supports (forms, interpretation, and internal referrals).

· Facilitates Client transfers when requested.

· Maintains records, forms and electronic encounter data as required

· Provides back-up support for the front desk and colleagues as required

· Other duties and responsibilities assigned as needed.

Client Advisor, Support and Education in the community

· Provides clients with support services, e.g. external referrals and information regarding community services, shelters, government services, partner agencies; assists clients with access to items such as clothing, food and needle exchange

· Provides occasional accompaniment to services e.g. emergency room, specialist appointments, shelters or other agencies

· Provides information on internal and external resources as required

Health Promotion

· Ensure appropriate care is delivered by maintaining client records, assessment notes and correspondence within the safeguard of confidentiality.

· Supports in identifying and addressing local health and social issues that impact the health of residents, through research, community engagement, etc.

· Supports the design, implementation and evaluation of health promotion programs and activities to support clients and community members.

· Engage community groups and networks in support of community initiatives and represent the organisation at meetings with those groups.

· Identify assets and resources that can be used to build a healthier community

· Identify and promote activities,



tools and resources related to health literacy, cultural competency, health equity, and anti-oppressive practices

Qualifications and Experience

· Must have a minimum of Social Service Worker diploma

· Significant experience in client coordination and group facilitation.

· Minimum of two years’ experience working for a community-based health/services and/or values-based organization.

· Knowledge of health and social service providers, their policies, and support networks

· Familiar with community resources and government services.

· Ability to establish good working relationships with partner agencies that provide the continuum of health and social sector services to clients.

· Excellent written communication and oral skills that meet the needs of the individual and/or community at the appropriate literacy level.

· Experience working with confidential and time-sensitive documents and information.

· Excellent interpersonal skills and the ability to multitask in a fast-paced environment.

· Experience working in a value-based non-profit or social services organization.

· Demonstrated ability to work independently and interdependently in an interdisciplinary team environment in which the skills and responsibilities of each team member are recognized and respected.

· Demonstrated proficiency in the use of computers and various computer software applications, such as Electronic Medical Record (EMR).

· Ability to speak languages relevant to the residents of the organization’s community is an asset.
