Position Summary

The Service Coordinator plays a key role in supporting residents at the Temporary Transitional Housing (TTH) by coordinating services, supporting individualized care and support planning, and ensuring accurate documentation and communication. This position works closely with residents, internal staff, and external service providers to promote stability, safety, and wellness using trauma‑informed, harm‑reduction, and culturally responsive approaches.

Service Coordinators provide consistency, organization, and follow‑through within programs, bridging direct service delivery and program coordination while upholding YKWS policies, values, and expectations.

Key Responsibilities

Resident Support & Coordination

- Sit with residents to collaboratively develop, implement, and regularly review individualized care and support plans.
- Lead care and support planning in a resident-centred, trauma-informed, and culturally responsive manner.
- Support residents with intake, orientation, and transitions within programs.
- Coordinate access to community services including health care, income support, housing, legal, cultural, and wellness supports.
- Accompany residents to appointments, meetings, and other community-based activities as part of their care and support plans (e.g., health care, income support, housing, legal, cultural, and wellness services).
- Develop and support behavioral wellness plans when residents are experiencing challenges with program expectations.
- Maintain professional, respectful, and trauma-informed relationships with residents at all times.

Documentation & Reporting

- Complete, update, and maintain accurate resident files, including care and support plans, consent forms, incident reports, wellness plans, and related documentation.
- Ensure documentation is completed in a timely, clear, and professional manner and aligned with YKWS standards.
- Enter and maintain required data in HIFIS and other tracking systems as required.
- Support audits, reporting,



and program reviews as requested by management.

Program & Team Support

- Work collaboratively with Resident Support Workers, Support Workers, and Program Managers to ensure consistent service delivery.
- Participate in shift handovers, team meetings, case conferences, and care planning discussions.
- Support program operations by identifying gaps, trends, and emerging resident needs.
- Assist with unit checks, wellness visits, and inspections as required by the program.

Safety & Policy Compliance

- Uphold YKWS policies, procedures, and program expectations at all times.
- Respond appropriately to incidents, emergencies, and crises, following established protocols.
- Maintain confidentiality, professional boundaries, and ethical conduct in accordance with YKWS policies.
- Promote a safe, respectful, and inclusive environment for residents, staff, and visitors.

Cultural Safety & Trauma‑Informed Practice

- Apply trauma‑informed, harm‑reduction, and Indigenous‑focused practices in all aspects of the role.
- Demonstrate cultural humility, respect for Indigenous peoples, and awareness of the impacts of colonization and intergenerational trauma.
- Support resident self‑determination, dignity, and choice.

Qualifications

- Diploma or degree in Social Work, Human Services, Addictions, Community Support, or a related field; or equivalent combination of education and experience.
- Minimum 2 years of experience working in housing, shelter, social services, or a related frontline environment.
- Experience with case management, care planning, or service coordination is a robust asset.




- Knowledge of trauma‑informed care, harm‑reduction, and housing‑first principles.
- Experience working with Indigenous populations and an understanding of northern and community‑based services is highly valued.
- Proficiency with Microsoft Office and electronic record management systems.
- Valid Class 5 Driver's Licence is required.
- Recent Vulnerable Sector Check are required.

Working Conditions

- This positions work site is located out at the Folk on the Rocks Site.
- Work takes place both indoors and in the community.
- Regularly accompanies residents to appointments, meetings, and community-based services.
- May involve walking, driving, or using public transportation to support residents in the community.
- Frequent interaction with individuals experiencing crisis, trauma, or complex needs.
- Requires emotional resilience, sound judgment, and adherence to safety and lone-worker protocols.
- Work may include exposure to unpredictable environments and requires situational awareness and flexibility.
