The West Durham Family Health Team (WDFHT) is seeking a full time temporary Occupational Therapist (OT) to join its team of primary care professionals. The OT will deliver Occupational Therapy services for individuals, families and caregivers, with a primary focus on the geriatric population.

Summary of Main Responsibilities:

Patient Education:

· Educate and counsel patients and caregivers regarding the impact of disability or disease

· Educate and counsel patients and caregivers to promote function and independence including health promotion and injury prevention

· Counsel patients regarding activities of daily living and interventions to support life skills maintenance

· Develop and facilitate group-based self-management programs as needed

· Support caregivers of patients with a disability, chronic disease, etc.

Assessment:

· Conduct assessments and interventions related to falls, balance, home safety, etc.

· Identify and address psychological, social and/or environmental factors that can affect functioning

· Assess for varying assistive devices and applicable funding available

· Conduct home and/or community safety assessments as required

· Perform assessments of cognitive and functional impairment

· Perform Psychosocial assessment / treatment

Treatment and Management:

· Address accessibility and mobility issues

· Assist patients and their caregivers with applications/forms to link them with resources and/or funding

· Provide training and techniques, share knowledge to promote independence

· Implement and monitor treatment plans and programs including but not limited to home modifications, mobility devices, falls prevention, caregiver support,



education and community linkages

· Collaborate with members of the interprofessional team to deliver comprehensive care and service/system coordination; refer patients to appropriate internal and/or external resources

Qualifications/ Job Requirements:

· Graduate of an accredited school of Occupational Therapy

· Current Active registration with the College of Occupational Therapists of Ontario

· 3-5 years of experience in healthcare, preferably in a community setting

· MOCA certified

· Strong clinical and case management skills related to establishing, setting, treating and monitoring patients’ functional goals

· Proven efficient and effective patient care and ability to leverage existing resources

Pay: From $74,000.00 per year
