: The overall function of the assessor/coordinator (client care coordinator) position is to assess and coordinate single entry access to personal care homes and long term care. "Preference for this position will be given to HSAS members in accordance with the terms and conditions of the SAHO/HSAS Collective Agreement"

Human Resources Exemption: No

Education

- Bachelor of Science in Nursing (BScN)
- Bachelor of Social Work (BSW)

Other Education and Training

- Bachelor of Nursing is acceptable
- Bachelor of Social Work is acceptable

Knowledge and Abilities

- Knowledge in the SHA, community resources and applicable programming

Other Information

- Registered/eligible with the Saskatchewan Association of Social Workers (SASW)
- Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)
- Valid driver's license, where required
- Travel required
- Previous Client Assessment Experience is an Asset
