Detailed Overview
Supporting the Vision, Values, Purpose and Commitments of Fraser Health including service delivery that is centered around patients/clients/residents and families:
This position is responsible to prioritize, coordinate and facilitate the appropriate placement of patients requiring cardiac surgery within Royal Columbian Hospital (RCH) and throughout Fraser Health.
In direct collaboration with Cardiac Surgeons and the RCH Surgical Program, is accountable to lead the development, implementation, evaluation and maintenance of a cardiovascular waitlist management system and database to prioritize and facilitate patient placement. Establishes systems to advise/notify cardiologists and referring physicians of patient health status to facilitate completion of procedures within wait-time guidelines; provides patient/family-centered support and education; acts as key liaison for patients and medical staff; ensures the integrated tracking and reporting of statistics related to quality indicators, outcomes, program effectiveness and quality improvement. Develops policies and procedures appropriate to various areas such as patient preparation, efficient transfer of patient cases, tracking of complications, and development of appropriate avenues for follow-up.
Responsibilities
1. Establishes systems and processes to assist referring physicians in triage of patients for pacemakers, ICDs (implantable cardiac defibrillator), CRT's (cardiac resynchronization therapy), cardiac catheterization, and cardiac interventions.
Ensures referrals are processed in a timely manner and includes complete urgency status, clinical, demographic and contact information.
2. Develops, implements and manages an effective cardiovascular waitlist management system for RCH and Fraser Health. Collaborates with Cardiologists, Internists, other Cardiovascular Coordinators and the British Columbia Cardiac Registry in the development, implementation and maintenance of a current, complete and accurate database for all patients awaiting cardiac surgery. Ensures database includes: tracking of patient waiting times and stability while awaiting various procedures related to cardiac intervention and diagnostic catheterization. Maintains current waiting lists, identifying patients exceeding recommended maximum waiting times. Ensures statistics for waiting lists and cases completed are complete and recorded in the database.
3. Facilitates continuity of flow for cardiovascular patients from initial booking to post-discharge rehabilitation and follow-up care. Ensures that all appropriate pre-operative diagnostics are complete; appropriate patient results, consultations, and procedure reports are available to the on-call interventionist; coordinates discharge planning. Coordinates transfer of post-operative patients to other hospitals as necessary.
4. Establishes systems for notification for cardiologists and referring physicians of changes in patients' conditions;. Collaborates with physicians to develop options to facilitate patients having procedures completed within recommended waiting time guidelines. Develops reports for physicians and the interventional cardiology department related to database inquiries.
5. Ensures that patient/family centered support and educational information is provided to cardiovascular patients in a timely manner. Education includes: informing patients/families of available options for procedures within wait time guidelines; procure information, facilitating referrals to required services (ie counseling, cardiac rehabilitation).
6. Acts as a key liaison for external and internal providers and patients; provides pertinent information to stakeholders to facilitate decision-making and timely access to care.
7. Coordinates triage rounds to ensure most urgent cases are prioritized; attends rounds and records the plan of care for each patient. Disseminates plan of care post rounds in discussions with the team. Collaborates with cardiologists to develop criteria for cardiac surgery consults.
8. Addresses system gaps impacting quality of patient care such as pre-admission patient education, communication of wait list status information to physicians, ongoing telephone support to wait- listed patients.
9. Develops and implements a joint triage system with an appeal process for the cardiologists to ensure timely case review and patient selection.
10. Collaborates with the Director of the Cardiac Registry and other Cardiovascular Coordinators to integrate tracking of quality indicators and outcomes.
Prepares and presents monthly statistics to the cardiac program team; responsible for quality improvement projects to optimize program effectiveness and efficiency.
11. Develops policies and procedures re: patient preparation, transfer of patient cases, tracking of complication s and development of appropriate avenues for follow-up and other related areas.
12. Provides work direction to a variety of clerical staff as appropriate.
13. Represents the dept on internal and external committees as appropriate.
Qualifications
Education and Experience
Undergraduate degree in a health discipline such as nursing, cardio-technology, perfusion, health administration, or related clinical discipline, plus a minimum of five (5) years' recent related cardiac experience in a complex tertiary care acute environment, supplemented with experience in research/patient registry databases, report writing, quality improvement measurement and program development; or an equivalent combination of education, training and experience.
