- Perform scheduled visits to assess patient needs
- Deliver safe, ethical nursing care at homes
- Utilize assessment tools to determine care plans
- Adjust care strategies based on continuous monitoring
- Foster robust rapport with patients and families

Requirements:

- Current CNO registration as an RPN
- Access to a vehicle with a valid driver’s license
- Experience providing community care preferred
- Flexibility and reliability to meet patient needs
- Ability to travel within the designated area
