In this role, you will lead the resident transition process and collaborate with the inter-skilled team to assess individual needs.

Providing counseling and service plans will directly improve residents' psychosocial well-being while addressing care planning and communication with families every step of the way. Key Responsibilities:

- Guide residents through the moving process with support
- Offer social work counseling to meet psychosocial needs
- Plan and evaluate individualized social work services
- Facilitate advanced care planning discussions
- Track and respond to resident feedback effectively

Requirements:

- Must be a Registered Social Worker in your province
- At least three years in healthcare or social services
- Familiarity with Long Term Care Act preferred
- Excellent oral and written communication skills
- Experience in customer service or hospitality is beneficial
