Department: Assertive Community Treatment Team (ACTT) - Community Mental Health & Addictions Site The Assertive Community Treatment Team (ACTT) is a best practice model providing specialized community care supporting people living with severe and persistent mental illness in achieving satisfying, hopeful and contributing lives. The teams provide community-based client-centered, recovery-oriented mental health services for persons with serious mental illnesses. Services are delivered by an interdisciplinary mental health team that provides the majority of the treatment, rehabilitation, and support services that clients need to achieve their goals.

Intensive services are available 24 hours per day, seven days a week and are provided based on individual needs and a mutually agreed upon plan between the client and ACTT staff. ACTT services are individually tailored to each client, taking into account their preferences, needs, and identified goals.

QUALIFICATIONS

Certification/Experience/Knowledge:

- Bachelor of Social Work (BSW) required, Masters in Social Work (MSW) an asset
- Current certification of Registration from Ontario College of Social Workers and Social Service Workers (OCSWSSW) required
- Current Basic Life Support (BLS) for Health Care Providers (HCP) Certificate required (recent within the last twelve months)
- Current Gentle Persuasive Approach (GPA) Certificate an asset
- Thorough knowledge and practical experience with theories and practices of professional social work within the addiction and mental health field
- Knowledge of the Mental Health Act and related legislation




- Knowledge of best practices in addiction and mental health crisis intervention, the Patient Family Partnered Care competencies and practices and the Assertive Community Treatment Team model of care
- Advanced knowledge of relevant community resources and service systems to facilitate support for clients and significant others through the process of community integration, recovery, and advocacy
- Knowledge of social work treatment interventions and best practices applied in recovery, clinical venues and community settings which incorporate psychosocial rehabilitation values and principals in the client’s treatments
- Demonstrated knowledge of community mental health, addictions, and vocational services
- Knowledge of psychiatric conditions, psychotropic medication, related side effects, symptom management and teaching an asset
- Demonstrated knowledge and experience in applying a Recovery Based Philosophy in mental health an asset

Skills/Abilities:

- Demonstrated analytical and critical thinking skills
- Ability to work both independently and collaboratively with all members of the multi-disciplinary team, clients, families, regional support services and community partners.




- Excellent verbal and written communications skills required
- Valid class “G” Ontario Driver’s License and Ministry of Transportation Rivers’ Abstract for extensive travel within the catchment area
- Computer skills required
- Must demonstrate commitment to ensure their safety, the safety of fellow workers, patients, volunteers, and visitors
- Must support and contribute to a culture of safety and prevention of adverse health events in this organization
- Good attendance and work record is required
- Bilingualism (Advanced level English/French) is required.
