Nurse Practitioner - Long Term Care (Temporary Full-Time) Job Posting Long Term Care (LTC) Temporary Full-Time (Approx.
Shifts
Days, Monday-Friday, 5 days per week, 7.5 hour shifts. The primary role for the nurse practitioner is to direct, provide and evaluate holistic client care for a specified client population. The nurse practitioner provides leadership to promote program development, process improvements and the enhancement of skilled standards.
The nurse practitioner acts as a clinical resource for nurses and other Regulated Health Care Professionals. Completes comprehensive health history and physical assessment. Identifies and orders diagnostic tests and procedures according to approved medical directives within the scope of practice.
Collects, interprets and analyzes data from multiple sources and uses this data to formulate medical diagnoses within scope of practice. Prescribes pharmacological and non-pharmacological therapies according to approved medical directives. Initiates interventions for the purpose of stabilizing clients in urgent and emergent situations.
Develops cohesive working relationships with interdisciplinary providers in our community, both internally and externally. Participates with nursing administration in the hiring, supervising and evaluation of nursing staff. Participates and provides leadership in the strategic planning process for the clinical program(s).
Demonstrates leadership skills, expertise, and in-depth knowledge base consistent with advanced nursing practice. Implements change using relevant theory and data by applying quality improvement principles. Utilizes principles of the consultative process to effectively communicate and contribute to the development of systems related to client care, organization and health care outcomes.
Initiates and collaborates in the strategic planning for program development, implementation and evaluation across the organization for specific client population. Provides and critically analyses data relevant to system/program decision-making.
Participates in local, regional, provincial, national and international forums to develop policy and standards.
Influences nursing practice by stimulating critical thinking. Analyzes and interprets current research / literature / knowledge applicable to health care practice with the identified client population. Advanced knowledge and decision-making skills in assessment, diagnosis and health care management.
Knowledgeable about primary health care issues in Long Term Care. Three years nursing experience as a Primary Health or LTC Nurse Practitioner in specialty area preferred. Excellent communication skills (verbal and written) and team building skills.
Excellent interpersonal skills and the ability to work co-operatively and constructively within an inter-professional healthcare team. Proficiency in Windows Operating System, Microsoft Office Suite: Word, Excel, Outlook, PowerPoint. Knowledge of project management and adult education.
Research and program evaluation skills an asset.
