POSITION: Nurse Practitioner (NP)

STATUS: RN (EC) – Full Time Position.

LOCATION: In office, In-person always: Hybrid or remote are not an option

LOCATION: Downtown Toronto: Rekai Centre at Wellesley Central Place Place

Seeking a Nurse Practitioner for an in-person, on-site position at Wellesley Central Place. This is a 150 bed Long Term Care Home located in downtown Toronto. This is a non-union, Management position.

Education and Specific Job Requirements

Required

- Legally entitled to work in Canada.
- Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class Primary Health Care or Adult and entitle to practice with no restrictions/limitations AND/OR Nurse Practitioners' Association of Ontario (NPAO)
- CPR Designation designed for HealthCare Professionals and Standard First Aid
- Advanced knowledge and decision-making skills in assessment, diagnosis, and health care management and treatment of older persons with delirium, depression, cognitive impairment, confusion and behavioural disturbance

Considered an Asset

- Certification in Geriatric Nursing from the Canadian Nursing Association or equivalent experience
- Certifications obtained or in progress that include gerontology, mental health, behavioral support and palliative care.
- Working Knowledge of the; Fixing Long Term Care Act, Program Standards for Long Term Care, Health Disciplines Act, Occupational Health & Safety Act, Pharmacy Act, Food and Drugs Act (Canada), Narcotic Control Act (Canada) and Coroners Act
- Certifications obtained or in progress that include gerontology, mental health, behavioral support and palliative care.
- Current membership in professional associations: Nurse Practitioners' Association of Ontario (NPAO) is preferred.
- Teaching experience within the Nurse Practitioner scope

Relevant Experience

- Two-Three (2-3) years’ clinical experience as a Nurse Practitioner in Canada
- New Graduates of Canadian recognized Nurse Practitionar Programs are welcome to apply

Working Environment: working in close proximity to individuals exhibiting responsive behaviours related to diagnosed neurological conditions, compounded by mental health disorders and substance use issues

Responsibilities:

1. Resident Assessment & Clinical Care

- Conduct comprehensive assessments of residents, including physical examinations, health histories, changes in condition, and ongoing clinical needs.
- Develop, implement, and evaluate individualized care plans that reflect residents’ needs, goals, preferences, and overall quality of life.




- Provide direct clinical care within the Nurse Practitioner scope of practice, including diagnosing and managing health conditions, ordering and interpreting diagnostic tests, prescribing medications, and performing minor procedures as appropriate.
- Provide timely assessment and management of acute, chronic, complex, palliative, and end-of-life care needs, including supporting appropriate alternatives to hospital transfer where clinically appropriate.

2. Clinical Leadership & Consultation

- Provide clinical leadership, consultation, and support to nursing staff regarding complex resident care, changes in condition, clinical decision-making, and evidence-informed interventions.
- Participate in care conferences, interdisciplinary meetings, that promote and support continuity of resident-centred care and optimal resident outcomes.

3. Medication & Chronic Disease Management

- Participate in medication reviews, medication optimization and deprescribing initiatives, and monitoring of high-risk medications in collaboration with pharmacy and the interdisciplinary team.
- Support the ongoing management of chronic conditions and changing health needs through evidence-informed clinical practice.

4. Goals of Care, Palliative & End-of-Life Care:

- Support advance care planning, goals-of-care discussions, palliative approaches to care, and end-of-life care in collaboration with residents, substitute decision-makers, families, and the interdisciplinary team.

5. Resident, Family & Care Partner Engagement

- Educate and support residents, families, substitute decision-makers, and care partners regarding health promotion, disease prevention, treatment plans, self-management, and changes in health status.
- Promote resident choice, dignity, autonomy, and quality of life in all aspects of clinical care.

6. Quality, Safety & Infection Prevention:

- Participate in quality improvement, resident safety, infection prevention and control, and outbreak management initiatives as required.

7. Documentation, Professional Standards & Regulatory Compliance

- Maintain accurate, timely, and comprehensive documentation of assessments, clinical interventions, care plans, and outcomes within the electronic health record (EHR).




- Maintain clinical practice in accordance with the standards and guidelines of the College of Nurses of Ontario, applicable legislation, regulatory requirements, organizational policies, and evidence-informed long-term care practices.

8. Education, Mentorship & Learning:

- Mentor and support nursing students and other learners by providing role modelling professional practice. Share knowledge and support the development of nursing staff through clinical consultation and education, as appropriate.

9. Person-Centred & Inclusive Care:

- Support the Home’s Emotion-Based Care approach and initiatives that promote an inclusive, equitable, and respectful environment, including the 2SLGBTQ+ initiative.

10. Other Responsibilities:

- Participate in other clinical, educational, quality improvement, or organizational activities as assigned in collaboration with the interdisciplinary team.

Skills, Knowledge Requirements

General

- Strong leadership, clinical judgment, critical thinking, organizational, and communication skills.
- Advanced health assessment, diagnostic reasoning, and clinical decision-making skills with a high degree of accuracy and attention to detail.
- Knowledge of gerontology, chronic disease management, dementia care, responsive behaviours, palliative care, and health promotion in a long-term care setting.
- Ability to build and maintain effective working relationships with residents, families, substitute decision-makers, care partners, the interdisciplinary team, and external partners.
- Ability to work effectively within a collaborative practice model and provide clinical guidance, education, and support to staff and learners.
- Ability to prioritize competing demands, manage time effectively, and respond appropriately in a fast-paced and changing work environment
- Self-motivated, versatile, and able to adapt quickly to organizational and resident care needs.

Computer Skills

- Proficiency with electronic health records and clinical documentation systems used in long-term care, including PointClickCare or related platforms.
- Working knowledge of Microsoft Office applications and ability to use digital tools for documentation, communication, reporting, and care coordination.

Language Skills: Excellent verbal and written communication skills in English are required; proficiency in an additional language is considered an asset.

Reporting Structure The NP reports to the Executive Director for day-to-day operations. Collaborate with the Director of Care (DOC) and Medical Director (MD) to help support and align with our resident care needs.
