Fife House is an cutting-edge, client-focused provider of secure and supportive affordable housing and services to people living with HIV/AIDS in the Greater Toronto Area. With approximately 110 staff working across 6 locations, in 2020-2021, Fife House served more than 850 residents and clients by providing supportive and transitional housing programs as well as outreach programs to homeless clients and their families.

Fife House is seeking a dedicated individual to fill the Relief Personal Support Worker role within our programs. As a member of the team, the Relief Personal Support Worker is responsible for providing trauma-informed, client-centered support to individuals affected by trauma, while applying harm reduction principles and practices. The successful candidate will demonstrate self-awareness, compassion, resilience, and a commitment to ongoing learning, supervision, and the community process of reconciliation.
In this role, the Relief Personal Support Worker is expected to follow all Fife House policies and procedures and work a flexible schedule, as discussed and agreed upon with their supervisor. This position is based at 339 George Street, Toronto, Ontario.
DUTIES AND RESPONSIBILITIES:
● Bathing and oral hygiene:
- Bed bath, sink bath, tub bath or shower
- Assisting with oral hygiene by cleaning dentures, brushing teeth and /or gums and providing additional mouth care as necessary
- Assist with or provide hair washing as required
- Provide peri-care in accordance with the continence needs of the client
● Dressing:
- Assist with or dress the client according to his/her choice of clothing and timing
- Assist with bedtime care of the client
- Assist with the care and storage of client clothing
● Toileting:
- Assist client to the bathroom, including ensuring safe and direct route when client and family may have rearranged the client's room
- Provide an empty commode, bedpan or urinal as requested and appropriate and in a timely manner
- Provide colostomy care
- Empty catheter
● Transferring and Repositioning:
- Assist with safe lifts and transfers as identified in the resident care plan and respect the lifts policies
- Outline turning and positioning as required
- Mechanical lifts must be used when the need has been identified in the client’s care plan – all mechanical lifts require the presence of two staff members
● Feeding:
- Assist in the delivery, set-up and feeding of clients as outlined in their care plan
- Monitor the client for difficulties chewing and swallowing
- Encourage and assist the client by recognizing likes and dislikes and communicating these to the client’s support team so they can be reflected in the care plan
- Provide oral care /denture care after meals and as required
- Simple meal preparation as required
● Psych-Social and spiritual support:
- Respects clients' wishes, beliefs and spiritual practices
- Moderates approach based on client preferences and mood
- Care of the Fife House, Supplies and Equipment
● Follow policies, procedures and protocols to:
- Maintain a tidy and safe workplace, including engaging in appropriate infection control practices
- Laundry: including all tasks involved in the preparation and operation of the laundry for the facility and occasional inclusion of resident items
- Stocking of resident rooms for supplies and linens
- Terminal care of apartment after discharge or death
- Cleaning and sterilization of equipment as required
- Participation in fire drills and mock emergency drills
● Communication:
- Reporting changes in client status or safety concerns to the supervising Program Manager, RN/RPN or such other designate
● Other
- Coordinate care with external supports as required
- Participates in the orientation and ongoing education of Fife House staff, students and volunteers
- Complies with all the company policies and Procedures, Infection Control protocols and Occupational Health and Safety policies
- Participates in bedside rounds light housekeeping duties such as room cleaning and laundry support
- Life skills coaching and support Activities of Daily Living (ADL) & Instrumental Activities of Daily Living (IADLS)
- Reporting changes in client behaviour and condition
- Documenting of care and client goals/preferences
- Participate in non-violent crisis intervention
- Participate in client case management and care planning
- Participate in the ongoing client assessment process
- Support team delivery and updating of individual client service and care plan
WORKING ENVIRONMENT: All employees working within shelter environments and residential supportive housing are required to remain awake, alert, and fully engaged for the entire duration of every shift. Staff support vulnerable populations, including individuals experiencing homelessness, mental health challenges, substance use, trauma, or other complex needs, and must be capable of responding to emergencies, crises, and resident needs at all times. Employees are expected to maintain regular communication with their supervisor and team during work hours and uphold a respectful, inclusive environment where racism, homophobia, transphobia, and all forms of discrimination are not tolerated.
Staff should understand that the role includes entering clients’ homes and units for wellness checks, follow-ups, and maintenance support, where conditions may include pests, clutter, strong odours, or second-hand smoke; occasional lifting of heavy items may be required, with safety equipment provided. Staff may interact with individuals experiencing mental health crises, substance-related psychosis, or drug poisoning, and will be expected to accompany clients from time to time independently in the community, work in all weather conditions, and use public transit or walk between work locations while following established safety procedures when necessary.
QUALIFICATIONS:
- Completion of Personal Support Worker Certification at an approved institution
- Completion of post-secondary education in either Social Service Worker Certification is required
- At least 1 year of experience providing personal support services
- First Aid and CPR certification
- Prior experience in ongoing assessment of needs using standardized assessment tools
- Prior experience with care, case management and service planning
- Knowledge of HIV infection in the current context and knowledge of the HIV sector in Ontario
- Experience and/or education in addictions, corrections and mental health
- Demonstrated and proven ability to work with a team and community partners
- Demonstrated ability to manage conflict and crisis
- Physically able to perform light physical work
- Demonstrated commitment to client-centred care
- Demonstrated ability to recognize safety concerns
- Good oral and written skills, excellent computer skills
- Understanding of the Residential Tenancies Act
ASSET:
- The ability to communicate in a different language
- Food Handlers certification
- Prior experience working in supportive housing
- Non-violent crisis intervention training
