Position Summary:
The Registered Nurse working at full scope of practice in partnership with family doctor’s nurse practitioners Midwives Public Health and Pediatricians supporting discharge planning within the Women’s Health and Newborn Care Program. Responsible for the day-to-day support of the program supporting new moms and babies ensuring that families have access to information and assessment within 24-48 hours from discharge from the hospital. The role includes assessment planning teaching newborn assessments and bloodwork follow up of obstetric and newborn patients and connecting new moms with any support that she needs.
Qualifications:
- Current registration with the College of Nurses of Ontario required
- BScN preferred
- NRP Certification required
- IBCLC preferred
- 3-5 years of clinical practice in Women’s Health and Newborn Care with current experience in a full scope hospital setting required
- Highly knowledgeable on qualified Nursing standards and hospital guidelines and procedures
- Exceptional interpersonal and communication skills with a proven track record of fostering collaborative partnership
- Committed to excellence in exceptional patient care and continual focus on quality improvement and results-driven outcomes
- Exceptional clinical assessment skills
- Demonstrate excellent critical thinking and problem-solving skills
- Demonstrate ability to work in a dynamic fast-paced environment
- Proficiency in electronic patient charting
- Evidence of ongoing professional development continuing education and leadership experience
- Demonstrated knowledge skills and judgement required to provide a full range of care and treatment in the clinical area of practice to patients/clients including those with complex needs high acuity and less predictable outcomes required
- Demonstrated ability to communicate (written and verbal) effectively with patients/clients and families required
- Demonstrated ability to practice collaboratively and communicate clearly with members of the interdisciplinary team to provide care for a full range of patients/clients required
- Demonstrated ability to set priorities and organize work in response to patient/client and unit needs
- Demonstrated strong leadership problem solving critical thinking team building and interpersonal skills required
- Good attendance and work record is essential
Responsibilities:
- Facilitates discharge planning for Newborns that meet the discharge criteria and arranges follow-up in the hospital-based clinic
- Performs weight checks jaundice assessment and blood work on newborns if required
- Ability to follow Newborns for up to 7 days after discharge
- Provides support with developing safe newborn feeding plans
- Provides information clients need to care for both themselves and their baby
- Assists with patient navigation and connecting families with community supports
- Help to build confidence in caring and supporting client’s goals
