Join WoodGreen as a Psychogeriatric Specialist in a dynamic hybrid setting. Support older adults facing mental health challenges while engaging in meaningful case management and counseling.
In this part-time role, you will offer psycho-social support, develop care strategies, and ensure smooth transitions between hospital and community settings. Working under the guidance of the Manager, Caregiver Wellness, your contributions will have a direct impact on the seniors’ well-being. Focused on trauma-informed care principles, this role promotes a compassionate and inclusive environment for the vulnerable population you serve.
Key Responsibilities:
• Perform comprehensive geriatric assessments for clients • Develop and manage individualized care plans • Deliver therapeutic counseling and crisis interventions • Facilitate access to community resources and services • Organize therapeutic group activities and support clinics
Requirements: • M.S.W. or equivalent graduate degree • Positive standing as a Registered Social Worker in Ontario • Minimum 3 years in mental health and addictions field • Experience with group facilitation and intervention • Strong understanding of older adults' issues
