UHN is looking for a Speech-Language Pathologist for the Transitional Care Program. This position requires working closely with a multi-disciplinary team to provide evaluations and treatment for patients facing complex health challenges. Involvement in teaching and assisting with coverage across clinical services is essential.
Key Responsibilities: • Assess adult patients’ communication and swallowing functions • Support interdisciplinary care meetings and rounds • Conduct educational classes for patients and families • Complete necessary documentation to support patient services • Provide coverage to other clinicians when needed
Requirements: • Master’s degree in Speech-Language Pathology • Registration with CASLPO mandatory • Preferred 2 years’ experience in hospital settings • Excellent verbal and written communication abilities • Flexibility to adjust in fast-paced environments
