The Clinical Case Manager manages a caseload to provide supports to youth in the Independent Options/Young Parent Program, including conducting assessments to ensure program alignment with youths’ needs, developing comprehensive care plans and monitoring and adjusting them on an ongoing basis. They are responsible for supervising and directing front-line support staff in their delivery of treatment case plans.

Duties and Responsibilities:

Develops, implements, monitors and evaluates the operations of the program, ensuring compliance as per organizational requirements, Policies and procedures, and best practices for client-centred care within a safe and therapeutic milieu;

Develops an awareness of each youth’s individual psychological, educational, physical, spiritual, and social needs and goals, and actively promotes these on behalf of the youth;

Guides and facilitates the implementation of strategies such as planned daily activities, coordinated treatment interventions, structured environments, and organized recreational and social activities;

Oversees and supervises the activities of staff including but not limited to hiring, training, directing, and monitoring. Responsible for staff scheduling to ensure proper coverage is maintained at all times;

Evaluates employee performance, conducting regular supervision and development of staff, including performance management, with the support of Human Resources where required;

Monitors the status of required staff licenses and certifications;

Acts as a liaison within multi-disciplinary teams, both internal and external to the agency regarding client needs and services-youth focused activities such as appointments with clinicians, CFS Social Workers,



medical appointments etc.;

Facilitates and participates in staff and case manager meetings to discuss program goals and milieu, clinical treatment practices and program management;

Provides guidance to support staff regarding the implementation of treatment plans including education and training, direct modelling and feedback;

Initiates/facilitates and participates in case conferences;

Helps to identify supports, both internal and external to the agency, including community resources, to support the treatment plan and youth’s development;

Participates in and contributes to assessing clients’ progress by following up on the implementation and re-evaluation of behavioural strategies within the interdisciplinary team;

Safely and effectively implements behaviour management strategies to safely de-escalate crisis situations;

Demonstrates an active commitment towards professional growth and advancement through participation in training, workshops, etc.;

Actively participates on committees both internal and external to the agency;

Other duties as assigned.

QUALIFICATIONS:

Post-secondary degree in Social Work is preferred – a combination of education and relevant working experience may be considered;

Minimum of four years working experience providing physical, psychological,



and social support to at-risk youth and /or other vulnerable populations from a trauma-informed perspective is required;

Minimum of one year of supervisory experience is required;

Demonstrated understanding of issues impacting youth presenting with mental health and other complex needs;

Demonstrated experience and competence working independently and as part of a multi-disciplinary team, preferably working across agencies/systems in the provision of coordinated care;

Mental health/crisis training certifications including NVCI (Non-Violent Crisis Intervention) and ASIST (Applied Suicide Intervention Skills Training) are considered an asset;

Knowledge of planning, implementation and evaluation of client treatment plans;

Must be comfortable and competent in crisis de-escalation and behaviour management;

Comprehensive knowledge of children/youth services and resources available in the community;

Valid First Aid/CPR is required;

Valid class 5 driver’s license, access to a reliable vehicle, and satisfactory driver’s abstract;

Satisfactory Criminal Record and clear Vulnerable Sector checks;

Explicit Child Abuse Registry, clear Adult Abuse Registry, and satisfactory Prior Contact checks;

Knowledge of Indigenous culture, or willingness to learn, including participating in cultural practices, programming and ceremonies alongside youth;

Must be available to participate on an on-call rotation;

Excellent verbal and written communication skills;

Demonstrated decision-making and problem-solving skills;

Working knowledge of computers and experience with Microsoft Office software.
