Job Title: Nurse Practitioner (NP) – Full Time
Responsible To: Executive Director
Accountability: The NP will function in close collaboration with all physicians and Allied Health Care Professionals within the Family Health Team. As a vital member of the patient focused team, the NP will provide clinical services in collaboration with all health care professionals. In addition to providing a broad range of assessment, treatment, education and consultation services (specified by their scope of practice) the role of the NP will work with Family Health Team members to provide core services and specialized programs. The NP’s role also includes partnering with the Executive Director and other professionals in the development of programs, resources and the creation of linkages within the Family Health Team and community.
Qualifications:
Current registration with the College of Nurses of Ontario as a Registered NP in the Extended Class
Registration with the Nurse Practitioner Association of Ontario (NPAO) preferred
Masters Degree in Nursing
Two to three years clinical experience as an NP (Preferred)
Must have professional liability insurance
Ability to work as a team member to meet the nursing care needs of patients
Demonstrated teaching ability
Experience with program development and evaluation
Effective verbal and written communication skills
Advanced decision making, problem identification and problem solving skills
Strong organization and time management skills
The ability to work as part of a team in a health care environment
Proficiency in the use of computer and information technology
Experience working with an Electronic Patient Record (preferred)
Experience working with a multi-disciplinary team in the delivery of Primary Health Care
Hours of Work:
Compensation of $125,476 to $127,987
Roles and Responsibilities:
The NP will practice to their full scope of practice as defined by the CNO / NPAO
Thorough understanding of the CNO and NPAO principles and standards of practice
Excellent critical thinking and problem-solving skills
Excellent interpersonal and communication skills, including written and verbal
Comprehensive knowledge of acute care and chronic disease management
Ability to demonstrate tact and discretion in handling confidential and /or sensitive matters
Assessment, interpretation of patient needs and history, and appropriate treatment plan
Maintain accurate patient records; update patient charts to document progress
Support patients as needed; educate, communicate, advocate, coordinate / navigate services, etc.
Collaborate and consult with community groups and other health care agencies to provide ongoing support to fulfill community health needs
