In this full-time contract role, you will work with the East Team to develop personalized service plans. You’ll support clients in fulfilling their CTO obligations and enhance their access to essential resources. The role emphasizes a strength-based approach to mental health, requiring relationship-building with clients and multidisciplinary teams for effective outcomes.
Key Responsibilities:
• Develop individualized service plans with clients and stakeholders
• Support clients in meeting CTO obligations and accessing resources
• Conduct assessments and provide supportive counseling
• Build respectful relationships with clients and their networks
• Advocate for clients with community partners and service providers
Requirements:
• Undergraduate degree or diploma in social services
• Minimum three years of direct service experience
• Knowledge of crisis intervention and recovery-oriented practice
• Valid Ontario Driver's License and vehicle access
• Ability to work independently and in teams
