Take the next step in your career as a Case Manager with the Community Outreach and Support Team at CMHA in Bracebridge. This contract role focuses on delivering comprehensive support for individuals with responsible mental illness.

As a member of COAST, you will engage in case management, psychosocial rehabilitation, and crisis intervention. You will be expected to assess and develop treatment plans under clinical supervision to enhance the lives of clients with severe mental health issues.

Key Responsibilities:
• Provide crisis intervention and immediate support to clients
• Facilitate case management for diverse client needs
• Work collaboratively within a community support framework




• Conduct thorough client assessments and follow-ups
• Promote cultural awareness in service delivery

Requirements:
• Relevant Bachelor’s degree in health science or social work
• One year of applicable experience in mental health settings
• Knowledge of the Residential Tenancy Act
• Proficiency in technology and communication tools
• Valid Ontario driver’s license for transportation needs
