Key Responsibilities:
• Maintain accurate resident assessments and care plans
• Implement care strategies and monitor outcomes
• Provide direct nursing and medication administration




• Coordinate with physicians and support staff
• Assist with resident transitions and emergencies

Requirements:
• Current LPN registration in relevant province
• Background in gerontology or long-term care
• Solid care coordination and critical thinking skills
• Current CPR and First Aid certification
• Willingness to complete a Vulnerable Sector Check
