In this role, you will act as a Pediatric Mental Health Navigator, focusing on care coordination, case management, and support for families with complex needs. Key Responsibilities:
• Perform psychosocial assessments and risk screenings
• Coordinate case management and transition plans

• Engage with multidisciplinary teams for effective support

Requirements:
• Completion of a bachelor’s degree in Social Work
• Active or eligible registration with the ACSW
• Minimum of 5 years experience with youth and families
• Required certifications in BLS-HCP and NVCI
• Robust interpersonal and teamwork capabilities
