Join Wesley as a dedicated Health and Wellness Navigator providing essential support to individuals facing homelessness in Hamilton and Brantford. This full-time position focuses on mental health and harm reduction within a agile team.

Reporting to the Manager of Community Housing Services, you'll engage with clients experiencing substance use and related challenges. Your role will be pivotal in developing tailored service plans and promoting a harm reduction approach to enhance client well-being and recovery. Your efforts will make a difference in the lives of those in need through direct intervention and supportive services.

Key Responsibilities:
• Complete comprehensive assessments and service plans
• Provide crisis management and harm reduction support




• Build rapport with participants through regular engagement
• Manage harm reduction supplies and training
• Liaise with community partners for housing pathways

Requirements:
• Minimum of three years in relevant social services
• Post-secondary degree in Human Services or related field
• OCSWSSW registration preferred
• Mental Health First Aid certification is an asset
• Valid Driver's license and access to a vehicle
