As an Intensive Case Management Substance Use and Mental Health Support Worker, you will provide personalized outreach services, life skills coaching, and facilitate communication between clients and healthcare professionals. Your contribution is essential in addressing the needs of individuals facing complex challenges while adhering to the values of health equity and social justice. Every interaction counts in fostering positive change.
Key Responsibilities:
• Engage directly with clients in community settings
• Assist in substance use treatment plans
• Conduct life skills training and support groups
• Monitor client progress and health status
• Collaborate with a multidisciplinary team for integrated care
Requirements:
• Diploma in community social services or similar
• Two years of related experience in mental health
• Knowledge in chemical dependency
• Certification in First Aid and CPR
• Conflict resolution and crisis management expertise
