Join Island Health as a part time Home Support CHW Supervisor, LPN. This role focuses on leading Community Health Workers in delivering compassionate care across multiple sites.

As a Home Support CHW Supervisor, you'll utilize your LPN credentials and extensive experience to ensure high standards in patient care. Training, supervising, and assessing Community Health Workers will be central to your responsibilities. You'll collaborate with an interdisciplinary team to implement effective patient care strategies and service plans.

Key Responsibilities:
• Oversee service and direct care practice by CHWs
• Train and evaluate staff on client care skills
• Provide nursing care based on LPN standards




• Conduct client assessments and monitor health progress
• Work collaboratively with interdisciplinary health teams

Requirements:
• Current registration as an LPN with BC College of Nurses and Midwives
• At least three years of community or residential care experience
• Experience in a supervisory role is an advantage
• Must have a valid BC Driver's License
• Ability to handle physical aspects of the role

Lead nursing and community support services while ensuring quality care for clients.
