Join The Health Centre as an Outreach Community Health Worker to enhance community health in Waterloo Region. Support vulnerable populations through targeted outreach, case management, and resource navigation.

This full-time permanent position focuses on assisting individuals experiencing homelessness and complex health issues. You'll collaborate with Primary Care Outreach teams and engage with clients using an anti-oppressive practice framework. Your efforts will help drive community health initiatives and support clients in overcoming barriers to health services.

Key Responsibilities:

- Build trust-based relationships with clients in need
- Coordinate client care plans with multidisciplinary teams
- Provide in office support through direct outreach services




- Maintain accurate health records in the Electronic Medical Record
- Facilitate knowledge transfer across the outreach team

Requirements:

- Post-secondary education in social services or related field
- Minimum three years of community development experience
- Strong communication and problem-solving skills
- Ability to work collaboratively in diverse settings
- Familiarity with health services in Waterloo Region
