Conduct and document thorough client assessments
Develop and communicate care plans with the healthcare team
Identify health education needs and provide guidance
Order and interpret diagnostic tests as needed
Facilitate referrals to specialists and health servicesRequirements:
Must be a registered Nurse with Ontario's College of Nurses
2+ years of experience with complex medical issues
Previous NP experience in outpatient settings is an asset
Proven ability to work independently
