Conduct psychosocial assessments by engaging with patients and families
Deliver grief and bereavement counseling to address emotional needs

Maintain comprehensive patient records and documentation
Coordinate care plans with interdisciplinary teamsRequirements:
Master’s degree in Social Work and recent clinical experience
Registered with Ontario College of Social Workers
Expertise in emotional health and crisis intervention
Ability to deliver impactful presentations and training
