Empower health in the Noojmowin Teg community as a Nurse Practitioner. This pivotal role focuses on primary care delivery that respects both traditional and up-to-date healing methods.In this role, you will provide holistic health assessments, diagnoses, and treatments, building therapeutic relationships while addressing diverse health needs. Your work will emphasize chronic illness management and community health improvement, collaborating closely with various healthcare specialists. The Nurse Practitioner position is dedicated to fostering comprehensive care infused with cultural sensitivity.Key Responsibilities:
Evaluate and manage diverse patient health needs
Implement care plans for chronic illness management
Promote disease prevention and health education
Collaborate with providers for integrated client care
Participate actively in quality improvement projectsRequirements:
Bachelor’s degree in Nursing & Nurse Practitioner Certification
Current registration with the Ontario College of Nurses
CPR Level C and First Aid Certification required
Experience in primary health care settings preferred
