In this critical role, you will report directly to the Care Manager and ensure quality patient care with a focus on mental health. Your responsibilities include overseeing interdisciplinary care teams and interacting with clients and families to develop and modify care plans as necessary. Successful candidates will be required to provide direct care and assist in timely referrals to various stakeholders.
Key Responsibilities:
• Coordinate care and assess client needs
• Develop and modify care plans collaboratively
• Interact with team and family for planning
• Ensure timely referrals to other care providers
• Provide direct patient care as required
Requirements:
• Accredited Psychiatric nursing education program
• Eligible for registration with CRPNA
• Current Basic Cardiac Life Support certification
• Valid driver’s license for work-related travel
• Experience in community nursing preferred
