Make a difference in patient journeys as a Social Worker at Windsor Regional Hospital, addressing their psychosocial needs and facilitating rehabilitation during their hospital stay. Work within a multidisciplinary team to optimize care delivery.

This Social Worker position focuses on both the Medicine/Surgery and Women & Children's services, prioritizing patient-centered care and community reintegration. Your proficiency in identifying community resources will support adults and children while collaborating with other healthcare professionals to ensure seamless transitions and ongoing support.

Key Responsibilities:
• Conduct specialized psychosocial assessments for patients
• Develop and implement discharge plans effectively
• Coordinate care with community partners and service providers




• Foster collaboration with healthcare teams for optimal outcomes
• Promote a safe and compliant work workplace

Requirements:
• Minimum 1-2 years experience in acute care discharge planning
• Bachelor's degree in Social Work, master's preferred
• Registration with the Ontario College of Social Workers
• Strong organizational and communication competencies
• French language skills considered a plus
