You will serve as a resource for medication-related inquiries, actively resolve drug therapy issues, and manage adverse reactions while adhering to confidentiality and practice standards.

Key Responsibilities:
• Enter medication orders accurately into Meditech
• Collaborate with clinical teams during rounds




• Review medication profiles for safety and accuracy
• Address drug therapy concerns with prescribers
• Follow up on adverse drug reactions and incidents

Requirements:
• Bachelor of Science in Pharmacy or PharmD
• Registration with Ontario College of Pharmacists
• Previous hospital experience preferred
• Excellent problem-solving capabilities
• Strong communication skills, verbal and written
