Join CBI Home Health as a Care Supervisor in Edmonton on a 12-month contract. Drive quality care and treatment for individuals with disabilities through effective supervision.

This full time position involves overseeing individual care plans and ensuring service alignment with clinical standards. You will facilitate assessments and maintain communication with all involved stakeholders to optimize treatment outcomes. Lead the way in offering personalized care while supporting managers in monitoring care and clinical documentation.

Key Responsibilities:
• Coordinate individual care and clinical assessments
• Liaise with relevant care stakeholders
• Develop and adapt individual care plans
• Review clinical documentation for compliance
• Participate in case conferences and meetings

Requirements:
• LPN, RPN, or allied health professional education in progress
• Standard First Aid and CPR Level C certification
• Clear Criminal Record Check required
• Unrestricted Driver's License and vehicle access
• At least 1 year supporting individuals with service needs
