You will facilitate assessments and maintain communication with all involved stakeholders to optimize treatment outcomes. Key Responsibilities:
• Coordinate individual care and clinical assessments
• Liaise with relevant care stakeholders
• Develop and adapt individual care plans
• Review clinical documentation for compliance
• Participate in case conferences and meetings

Requirements:
• LPN, RPN, or allied health professional education in progress
• Standard First Aid and CPR Level C certification
• Clear Criminal Record Check required
• Unrestricted Driver's License and vehicle access
• At least 1 year supporting individuals with service needs
