Key Responsibilities:
• Complete and update psycho-social assessments of residents
• Provide counseling to residents, families, and staff




• Participate in interdisciplinary care conferences
• Develop and review care plans with staff
• Assist in discharge planning for residents

Requirements:
• Bachelor's Degree in Social Work required
• Two years of related social work experience needed
• Current BC College of Social Workers registration
• Solid communication skills essential
• Proven ability to work collaboratively in teams
