In this Clinical Pharmacist position, you will be part of a dedicated health team at Calgary Foothills PCN, enhancing patient care through evidence-based practices. Key Responsibilities:
• Manage patients’ medication regimens within healthcare teams
• Perform medication reconciliations and assessments




• Educate physicians and nurses on drug therapies

Requirements:
• Bachelor’s degree in Pharmacy required
• Licensed by Alberta College of Pharmacy
• Additional prescribing authorization after hiring
