Vancouver Coastal Health is seeking a dedicated RN/RPN for the Complex Care Housing team. In this role, you will collaborate with an interdisciplinary team to offer versatile outreach services and support to individuals facing mental health challenges. You'll conduct assessments, prioritize care requirements, and facilitate client transitions through various services.

Key Responsibilities:
• Conduct comprehensive mental and physical assessments
• Provide crisis intervention and counseling services




• Collaborate with the interdisciplinary team on care plans
• Assist clients with accessing community support and services
• Participate in case consultations and team meetings

Requirements:
• Current RN/RPN registration with BCCNM
• Minimum of two years’ related experience
• Valid BC Drivers License and personal vehicle access
• Knowledge of mental health, substance use treatment, and crisis intervention
• Strong communication and collaboration skills
