This permanent full time role with Kingston Community Health Centres (KCHC) offers a chance to work independently and collaboratively with healthcare professionals. You will provide primary care to patients, develop specialty clinics, and address social determinants to ensure effective healthcare delivery. Your commitment to culturally appropriate care will be vital in serving Indigenous populations and those needing support in primary care settings.
Key Responsibilities:
• Provide primary care to both rostered and unattached patients
• Design and implement specialty clinics with community Physicians
• Assess clients and prescribe treatments and medications
• Maintain accurate records in the electronic medical record system
• Participate in team meetings and health program development
Requirements:
• Registration as Registered Nurse (Extended Class) in Ontario
• 3-5 years nursing experience in primary care
• Experience working with marginalized populations
• Knowledge of trauma-informed care principles
• Successful Criminal and Vulnerable Persons Check
