Join Access Alliance as a Nurse Practitioner delivering primary care to diverse communities in Toronto. This full time role focuses on health equity for vulnerable groups, including immigrants and refugees.
In this position with Access Alliance Multicultural Health and Community Services, you will become an essential part of the Primary Care Team. The role involves providing holistic primary care and collaborating with community partners to improve health outcomes. Your responsibility includes addressing complex health needs and ensuring continuity of care for various populations through tailored health approaches.
Key Responsibilities:
• Deliver assessments and referrals for primary care needs
• Manage client caseload and interpret diagnostic tests
• Implement health education and support for chronic conditions
• Engage in community partnerships to enhance health services
• Maintain detailed and accurate client health documentation
Requirements:
• Masters prepared RN (EC) with active Ontario registration
• Two years of NP experience preferred
• Knowledge of health issues faced by immigrants and refugees
• Strong understanding of cultural differences in healthcare
• Ability to collaborate effectively with a health care team
