Make a significant impact in the lives of residents at Bruyère Health as a Social Service Worker. This full time, on-site position focuses on enhancing social functioning and wellbeing for elderly individuals.
As a Social Service Worker reporting to the Manager of Resident Experience, you will be responsible for assessment and treatment within our Long-Term Care setting. Utilize your skills to support residents and their families throughout their care journey, from move-in to discharge. Collaborate with interdisciplinary teams to promote quality of life and provide critical advocacy for residents' needs.
Key Responsibilities:
• Provide support during residents' collaborative living journey
• Assist residents through changes in ability and lifestyle
• Enhance quality of life by respecting residents' dignity
• Coordinate care conferences and facilitate discussions
• Collaborate with community services for continuity of care
Requirements:
• Diploma in Social Service Work from an accredited college
• Registered with the Ontario College of Social Workers
• Experience with elderly and palliative care
• Bilingual in French and English is essential
• Knowledge of community resources and medical terminology
