Vancouver Coastal Health is hiring a Mental Health and Addictions Outreach Nurse (RN/RPN) for its Intensive Health Outreach Team in Vancouver, BC. Engage with individuals facing mental health and substance use challenges in a supportive environment.

In this role, you will assess client needs, provide crisis intervention, and collaborate with a multidisciplinary team to create effective care plans. Your efforts will directly contribute to improving the lives of individuals who are often experiencing homelessness or are in transitional housing. You will aid clients in accessing essential services and support.

Key Responsibilities:
• Conduct comprehensive assessments of clients’ needs
• Offer counseling to help clients engage with health services




• Provide crisis intervention in high-risk cases
• Collaborate in developing and adjusting care plans
• Assist clients in accessing medical appointments

Requirements:
• Current RN or RPN registration with BC College
• At least 2 years relevant experience
• Valid BC Driver’s Licence with vehicle access
• Knowledge of mental health treatment and crisis management
• Solid communication skills, both verbal and written
