In this full time role, you will report to the Senior Manager of Primary and Clinical Care. Your expertise will be instrumental in planning and coordinating programs that adhere to established policies while supporting day-to-day operations. Collaborate within multidisciplinary teams to meet the diverse needs of the community effectively.

Key Responsibilities:
• Manage implementation of Home and Community Care programs
• Supervise staff and enhance day-to-day service quality
• Promote client safety and care standards
• Evaluate and adapt services for community needs
• Facilitate teamwork across various disciplines

Requirements:
• Bachelor’s Degree or relevant College Diploma with three years’ experience
• Proven leadership and coaching capabilities
• Knowledge of palliative care practices
• High proficiency in Microsoft Office
• Experience working in interdisciplinary settings
