Do you enjoy building meaningful relationships and helping people feel supported, confident, and cared for? Are you someone who can connect with families, solve problems, and manage your day independently? If so, this Client Care Coordinator role could be the right fit for you.

You’ll spend much of your time in the community meeting with seniors and their families, conducting care consultations, developing personalized care plans, and ensuring our clients continue to receive exceptional service. You’ll work closely with our internal team to coordinate care, resolve concerns, and create a positive experience for the families we serve.

This is a rewarding, relationship-focused role for someone who enjoys working with people, being out in the community, and making a meaningful difference every day.

Responsibilities

Meet with prospective clients and their families to understand their needs and develop personalized care plans

Conduct care consultations and respond to new service inquiries

Complete regular quality assurance visits with existing clients

Update care plans as client needs change

Build strong relationships with clients, families, and Care Professionals

Introduce Care Professionals to new clients and help support successful care relationships

Work closely with colleagues to coordinate all aspects of client care

Respond to client and family concerns and work toward positive resolutions

Maintain accurate client documentation and complete required reporting

Prepare weekly, monthly, and quarterly performance reports

Participate in community events,



networking opportunities, and relationship-building initiatives as needed

Participate in our rotating after-hours on-call schedule, typically every 4th to 5th weekend and 1-2 evenings per week

What We’re Looking For

CCA designation or education/experience in health care, senior care, social services, or a related field

2+ years of relevant professional experience

Strong communication and relationship-building skills

Excellent organizational, administrative, and computer skills

Compassionate, professional, and solutions-focused approach

Self-motivated and comfortable working independently while also contributing to a team

Strong problem-solving skills and the ability to adapt when priorities change

Valid driver’s license, reliable vehicle, and willingness to travel within our service area

Flexibility to participate in a rotating after-hours on-call schedule
