Job Overview Vancouver Coastal Health is seeking a Mental Health and Addictions Outreach Nurse (RN/RPN) to join the Intensive Health Outreach Team (IHOT) in Vancouver, BC. The role is part of an interdisciplinary team that provides triage, outreach, and crisis intervention services for individuals experiencing mental illness, substance use, and/or chronic medical conditions, including those who are homeless or in transitional housing.
Responsibilities Conduct mental and physical status assessments and assess clients’ physiological, psychological, sociocultural, and spiritual needs to determine priority of care.
Provide counseling and assist clients in reducing repeated contact with and use of acute and community health services.
Assess high‑risk situations and provide crisis intervention.
Collaborate with the interdisciplinary team to develop, implement, and adjust client care plans.
Facilitate the movement of clients between acute care, community agency support sites, and designated health centres.
Participate in case conferences and meetings with team members.
Assist clients with attendance at needed services such as medical appointments.
Qualifications & Experience Current registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives.




Minimum of 2 years recent related experience working with individuals with mental health and substance use issues, or an equivalent combination of education, training, and experience.
Valid BC Driver’s Licence and access to a personal vehicle for business‑related purposes.
Broad knowledge of crisis intervention, counseling skills, mental health and substance use treatment, psychopharmacology, and community resources.
Demonstrated ability to conduct psychiatric assessments and provide treatment planning, counseling, crisis intervention, and case coordination.
CPR certification and basic computer literacy for electronic client care documentation.
Solid communication skills, both verbal and written, with clients, families, colleagues, physicians, and other health‑care staff.
Ability to establish workload priorities, adjust schedules for unexpected situations, work independently and collaboratively, and provide consultation and leadership in a team setting.
