As a Community Case Worker, you'll assess and manage casework while developing tailored support for participants. Solid advocacy skills and the capacity to handle mental health and addiction issues are crucial. You'll work independently while forming partnerships with community services to ensure effective reintegration.
Key Responsibilities:
• Implement and manage individualized support plans
• Assess risks and clinical suitability of participants
• Collaborate with community partners for service delivery
• Participate in workplace diversity and team development
• Travel between community locations and facilities as needed
Requirements:
• Post-secondary education in relevant fields
• 1 - 3 years of social service experience
• Intermediate First Aid/CPR certification
• A valid G Class Driver’s License
• Proficiency in both official languages is an asset
