Exciting opportunity at Bruyère Health for a Full time Geriatric Assessor focused on senior care. This role involves delivering assessments that enhance elderly clients' health and independence.

Reporting to the Manager of Outpatient Programs, you will conduct comprehensive assessments to determine the needs of frail seniors. Your work will be vital in recommending interventions that optimize health and safety. Collaborate with clients, caregivers, and community resources for effective outcomes.

Key Responsibilities:
• Provide detailed in-home geriatric assessments
• Collect and analyze data for program evaluation
• Work with community resources to support independence
• Create and share clear assessment reports
• Offer client recommendations for improved health

Requirements:
• Bachelor's degree in Nursing or related field
• Five years minimum geriatric clinical experience
• Valid driver's license with vehicle access
• French and English fluency required
• Registration with relevant professional colleges
