In this role, you will lead the resident transition process and collaborate with the inter-skilled team to assess individual needs. Providing counseling and service plans will directly improve residents' psychosocial well-being while addressing care planning and communication with families every step of the way.

Key Responsibilities:
• Guide residents through the moving process with support
• Offer social work counseling to meet psychosocial needs
• Plan and evaluate individualized social work services




• Facilitate advanced care planning discussions
• Track and respond to resident feedback effectively

Requirements:
• Must be a Registered Social Worker in your province
• At least three years in healthcare or social services
• Familiarity with Long Term Care Act preferred
• Excellent oral and written communication skills
• Experience in customer service or hospitality is beneficial
