You will assess needs, plan goals, navigate services, advocate, and coordinate wraparound supports in collaboration with Health and Social Services and Child Welfare teams.
Based in Michipicoten First Nation or Sault Ste. Marie, the role requires travel and occasional evenings or weekends.
