Covenant House is hiring a Follow Up Worker to support youth in Toronto. This full-time opportunity focuses on helping young people secure secure housing and develop essential life skills.

In this role, you will engage with youth to create tailored housing plans, provide necessary life skills training, and link them with supportive services in the community. The position requires effective communication and ability to document client progress across multiple systems to facilitate client success.

Key Responsibilities:
• Complete assessments and develop individual housing plans
• Advocate for clients in various systems including education and employment
• Offer life skills training focused on budgeting and housing maintenance




• Coordinate care with internal and external services
• Follow up bi-weekly with participants to deliver services

Requirements:
• Degree or diploma in child and youth work or related field
• Proficient in Microsoft Office 365 applications
• Understanding of youth homelessness and human trafficking
• Community resource knowledge relevant to youth
• Experience with crisis management and support services
