Key Responsibilities:
• Monitor and document resident health assessments
• Collaborate on multidisciplinary care planning




• Implement patient care interventions effectively
• Evaluate treatment modifications and outcomes
• Lead and mentor Health Care Aides as needed

Requirements:
• Approved Licensed Practical Nurse’s certification
• Current CLPNA registration and certification
• Minimum two years’ related experience
• Basic Life Support certification
• Valid Class 5 driver's license
