As the MDT Coordinator, you will facilitate case conferences and manage patient referrals in collaboration with Enfield GPs. This role highlights your organizational skills as you ensure all pertinent patient information is available for effective clinical decision-making. By supporting the Proactive and Community Matrons, you'll play a key role in service improvement initiatives for enhanced patient care.

Key Responsibilities:
• Oversee MDT meetings, ensuring all necessary information is available
• Update and manage the Community Matron’s mailbox and data reports




• Take minutes and track actions from MDT meetings
• Assist in staff rota management and communication protocols
• Support the implementation of clinical outcomes and patient plans

Requirements:
• Proven experience in administrative roles, preferably healthcare
• Proficiency in data entry and computer software like RiO
• Strong understanding of patient management processes
• Excellent organizational and communication abilities
• Commitment to maintaining confidentiality and data integrity
