We are looking for a dedicated Licensed Practical Nurse to assess and implement care for residents at Grace Manor and Supported Housing. You will lead healthcare aides, develop care plans, and engage with families and interdisciplinary teams. Your expertise will contribute significantly to resident health outcomes and care continuity.

Key Responsibilities:
• Perform initial and ongoing nursing assessments
• Assist in developing and executing individualized care plans
• Communicate effectively with healthcare team members
• Manage resident documentation accurately
• Promote a supportive and healing workplace

Requirements:
• Graduate of an approved Licensed Practical Nurse program
• Two years of relevant nursing experience preferred
• Active CLPNA registration required
• Certification in Basic Life Support
• Compliant with Criminal Record and Vulnerable Sector Checks
