Candidates should have at least three years of experience in community mental health, providing crucial assessments and case management. Your role will involve understanding the ACTT model to ensure effective care integration.

Key Responsibilities:
• Facilitate comprehensive mental health assessments
• Manage treatment plans tailored to clients' needs
• Offer crisis intervention and suicide risk management




• Collaborate within a multidisciplinary team
• Document clinical interactions efficiently

Requirements:
• Bachelor’s in Social Work; regulatory body registration required
• Three years of experience in community mental health
• Knowledge of the Mental Health Act
• Strong decision-making and interpersonal communication skills
• Valid Ontario Driver’s License and clean driving record
